Meridian Business Support isseeking a dedicated and experienced Care Coordinator to support the delivery of high-quality, person-centred care across the Suffolk area. If you are passionate about care and ready to take the next step in your career, wed love to hear from you.
Key Responsibilities:

  • Develop and implement personalised care plans, reviewing and updating them regularly.
  • Shadow new carers, conduct competency checks, and offer ongoing support.
  • Provide hands-on care in the community as needed.
  • Be the first point of contact for both clients and carers.
  • Report regularly to the Homecare Branch Manager on service delivery.
  • Maintain the dignity of service users and uphold high standards in their homes.
  • Work collaboratively within the care team to ensure a seamless service.
  • Commit to professional development through training, meetings, and supervisions.
  • Adhere to health and safety policies and procedures.
  • Represent the service with professionalism and integrity.
  • Handle sensitive information with the utmost confidentiality.
  • Promote independence, rights, and choices for service users.
  • Efficiently manage rotas, ensuring timely and clear distribution.
  • Train and assess staff in safe and effective medication management.
  • Conduct care reviews with service users to ensure their needs are fully met.
  • Participate in the on-call rota as required.
Skills & Attributes Required:
  • Minimum of 2 years experience working in a care setting with vulnerable people.
  • Willingness to undertake an NVQ or other relevant training.
  • Strong empathy and understanding of individual care needs.
  • Excellent time management and the ability to work well under pressure.
  • Confident in making clear, balanced, and timely decisions.
  • Collaborative and proactive approach to team and care planning.
  • Commitment to ongoing mandatory and specialist training.
