Opportunity: Care Coordinator

Unique Senior Care is seeking a Care Coordinator to join our friendly team, supporting our Warwickshire & Coventry branches. The role involves scheduling care hours, matching clients and caregivers to provide high-quality, person-centred care. The position offers ongoing development and career progression.

Contract details:

  • 40-hour contract
  • Salary: £27,800, rising to £28,650 after successful probation
  • Location: Warwickshire (near Stratford-upon-Avon) & Kenilworth
  • Must drive and have own car

Responsibilities include:

  • Ensuring regulatory and contractual standards are met and exceeded
  • Providing leadership and management to caregiving teams
  • Assessing clients and planning high-quality, person-centred care
  • Representing Unique Senior Care in the community and building partnerships
  • Supporting the Registered Manager with care plan writing
  • Business development and community outreach
  • Communicating effectively with caregivers, clients, and families
  • Managing caregiver and client schedules, including leave planning
  • Matching caregivers to clients based on skills
  • Maintaining accurate records and ensuring compliance
