Job description

Job Title: Care Coordinator

As a Care Coordinator, you will play a critical role in ensuring that residents receive high-quality, person-centred care. Working closely with the care team to coordinate and manage care for residents, considering their individual needs, preferences, and goals. You will be responsible for organising your shifts, administering and recording medication, liaising with health professionals, and updating the residents’ computerised care plans. Additionally, you will assist with the induction, training, and supervision of support staff.

  • Good leadership skills, effective communication, and team collaboration.

Education and Qualifications
  • NVQ3 in Health & Social Care or working towards it.
