We are looking for an Integrated Care Worker to join our East and South City Nursing Team.
We have an exciting opportunity to onboard a new member to our Cambridge East and South City Nursing team. Working with us, you will be fully supported to gain new skills and experience needed to deliver high quality care to patients in their own home or residential setting in and around the area.
We provide a service seven days a week, working closely with other community services and Primary Care Networks to provide high standard holistic care to the population of Cambridgeshire.
You will have the opportunity to develop yourself, with extensive learning and development opportunities.
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. As an Integrated Care Worker, you will work closely with the nursing team to follow a plan of care in delivering high quality, best practice care to adults with a range of complex health needs within their own homes.
