Greater Manchester Mental Health Nhs Foundation Trust is seeking a Registered Mental Health Nurse to join a specialist service for first episode psychosis as a Care Co-ordinator. You will work in a client-centred, multidisciplinary team delivering evidence-based interventions to support recovery.
The role involves coordinating timely interventions within the CPA framework, travelling mainly within the Bolton area, and contributing to assessments and care planning in line with clinical pathways.
This is an exciting opportunity for a Registered Mental Health Nurse to join our specialist service for people experiencing a first episode of psychosis as a Care Co-ordinator.
You will be joining a client centred, dynamic and friendly multi-disciplinary team, who prides themselves on delivering high quality, evidence-based interventions for those experiencing first episode psychosis.The purpose of the role is to reduce the stigma associated with psychosis, provide an early intervention to reduce the length of time that people remain undiagnosed and untreated and to offer support and guidance throughout the persons recovery journey.
Under line management and clinical supervision from within EIT, the post holder will maintain and co-ordinate timely and appropriate interventions, for a caseload of service users living in the Bolton area. The post holder will work within the framework of CPA, and in line with relevant clinical pathways as outlined within the EIT Operational Procedure.
