Active Care Group’s Care in the Home Division - Care Coordinator Role

Due to business growth, we are expanding our team of Care Coordinators and opening a new hub in Thame, Oxford!
Join our friendly team supporting our Care in the Home division. You will provide excellent customer service, coordinate and schedule care packages to ensure continuity of care for our clients at home.
Our services support children, young people, and adults with conditions such as brain injury, spinal cord injury, neurological and neurodegenerative diseases, respiratory and gastrointestinal conditions, including PEG feeding.
Working Hours:
- 37.5 hours per week - office based
- 9:00 am to 5:30 pm
- On-call rota: 1 week out of every 8
Responsibilities:
- Handle inbound and outbound calls with clients and workers to check welfare and shift coverage
- Update the Team Leader on critical team issues daily
- Maintain compliance with company policies and ensure regulatory standards are met, including safeguarding, complaints, and continuity of care
- Communicate effectively with staff, clients, and the public, handling complex and sensitive information
- Contribute to innovative ideas and support a flexible, creative workplace
- Ensure workloads are managed efficiently and best practices are followed
- Represent the company positively and develop stakeholder relationships
Requirements:
- Experience as a Care Coordinator
- Familiarity with booking and phone systems
- Conflict resolution skills
- Strong work ethic, flexibility, and teamwork skills
- Excellent communication and interpersonal skills
- Willingness to learn and develop
- Enhanced DBS clearance required
