Are you an experienced care professional with strong organisational skills, a compassionate approach and the confidence to coordinate high-quality care? We are seeking a Care Coordinator to join a dedicated care team in Manchester, supporting the delivery of safe, person-centred and consistently high standards of care.

This is an excellent opportunity for an experienced Care Coordinator, Senior Care Assistant, Senior Support Worker or experienced care professional looking to develop their career within a supportive and values-led environment.

THE ROLE

Working closely with colleagues, individuals receiving care and key stakeholders, you will help ensure care is coordinated effectively, safely and with the individual always at the heart of decision-making.

Key responsibilities include:
  • Coordinate and oversee the effective delivery of person-centred care and support.
  • Assist with care planning, reviews and the implementation of individualised support plans.
  • Promote dignity, choice, independence, privacy and respect at all times.
  • Support the effective allocation and coordination of staff and daily care requirements.
  • Maintain accurate, confidential and up-to-date care records and documentation.
  • Monitor care standards and identify opportunities for improvement.
  • Communicate effectively with colleagues, individuals, families and relevant professionals.
  • Support staff to understand and consistently follow agreed care plans and procedures.
  • Assist with medication processes and safe working practices in line with policy and individual requirements.
  • Contribute to safeguarding, risk management, incident reporting and quality assurance.
  • Support audits, inspections and regulatory compliance.
  • Provide guidance, encouragement and day-to-day support to care staff.
  • Respond professionally and compassionately to changing needs and emerging concerns.
  • Promote a positive culture of teamwork, accountability, inclusion and continuous improvement.
WHAT WE’RE LOOKING FOR

You will ideally have experience within adult social care, residential care, domiciliary care, supported living or a similar care environment , together with a genuine commitment to improving people’s lives.
