Position: Care Coordinator

Location: East Cheshire, Macclesfield

Salary: 28,000 per annum

Employment Type: Permanent

We are seeking a highly motivated and compassionate Care Coordinator to join our domiciliary care provider based in East Cheshire, Macclesfield. Our company is dedicated to providing high-quality care to individuals in their own homes, and we are looking for a candidate who shares our passion for making a positive impact on the lives of others.

Responsibilities:

  • Coordinate and manage care services for clients, ensuring timely and effective delivery of care
  • Conduct initial assessments and develop care plans for new clients
  • Monitor and review care plans on a regular basis, making necessary adjustments as needed
  • Communicate with clients, their families, and care staff to ensure all care needs are being met
  • Conduct regular supervisions and appraisals for care staff
  • Assist with recruitment and training of new care staff
  • Maintain accurate and up-to-date records and documentation
  • Participate in on-call duties as required
  • Requirements:

  • Proven experience in a care coordination role, preferably within domiciliary care
  • NVQ Level 3 in Health and Social Care or equivalent
  • Excellent communication and interpersonal skills
  • Strong organizational and time-management abilities
  • Ability to work independently and as part of a team
  • Knowledge of CQC regulations and standards
  • Full UK driving license and access to a vehicle
