Job description

This is an exciting opportunity for an enthusiastic and committed registered mental health nurse, occupational therapist, or social worker to work within Dulwich Community Mental Health Team (CMHT).

Dulwich CMHT is a multi-professional mental health team, comprising Social Workers, Occupational Therapists, Nurses, Support Workers, and Doctors. We are looking for a positive and motivated individual who understands the Trust’s values and their role in supporting and promoting recovery. This post is open to those professionally registered with the HCPC, NMC, or Social Work England. It is desirable for applicants to have experience working with patients in a mental health community setting.

Key Responsibilities:
  1. Working as part of a multi-disciplinary team alongside colleagues from various professional backgrounds.
  2. Undertaking assessments of need and developing sustainable recovery-focused care plans.
  3. Providing practical support and carrying out planned interventions to address assessed needs and overcome barriers to access and social inclusion.
  4. Using a strengths-based approach to support service-users in developing their own solutions, minimizing long-term dependence on services through self-directed support, and promoting the take-up of personal budgets where appropriate.
