Job description

Four Oaks Healthcare is a nurse-led provider of live-in and domiciliary care and, due to expansion, we are looking for an enthusiastic, passionate individual to join our growing team as a care coordinator.

As the care coordinator., you will be responsible for managing a team of care assistants as well as maintaining a good communication line with all customers and other parties to ensure we deliver a high standard of care.

The Ideal Candidate Will

  • Have a passion for delivering good quality, person-centred care
  • Ideally, have at least 2 years of experience within the care sector
  • Be flexible and reliable
  • Be computer literate
  • Be a good team player
  • Be motivated and able to work on their own initiative
  • Have exceptional people management skills
  • Must be a driver with access to own vehicle and willing to drive to other towns to meet clients (not a frequent occurrence)

Some Duties Involved

  • Preparing & scheduling rotas
  • Writing and reviewing care plans
  • Conducting risk assessments
  • Interviewing potential candidates
  • Conducting spot checks
  • Being on call, there is no call-out duties
