As a skilled Care Coordinator, you’ll already be used to managing a caseload of care packages and understand the varied nature and fast pace of the role, along with all elements of associated care delivery.
You’ll apply your gained communication and relationship-building skills to effectively manage rotas for your care teams, respond efficiently to any day-to-day changes, and act as the first point of contact for service users, their families, and other healthcare professionals.
No matter which care setting you’ve obtained your Care Coordination experience in, you’ll be aware that no two days are the same, so you’ll already be able to evidence your adaptability and aptitude to work well under pressure, using your initiative and thinking on your feet.

This is an office-based role, Monday to Friday, 9am – 5pm but at times, travel across the Northwest region will be required. Therefore, applicants must hold a Driving Licence and have access to a vehicle.
It is essential that you are committed to supporting the out of hours on-call service, which is covered on a 1 in 8 rota basis across the team.
We're offering a competitive salary between £26,000 - £28,000 per annum, plus payment for on-call. There is also the opportunity to earn an annual bonus of up to £2,000pa.
