Position: Care Coordinator

Location: Harrogate, UK

Salary: £28000 per annum

Type: Permanent, full-time

We are currently seeking a highly motivated and compassionate Care Coordinator to join our domiciliary care provider located in Harrogate. As a Care Coordinator, you will be responsible for coordinating and managing the delivery of care services to our clients in the community. The ideal candidate will have excellent organizational and communication skills, as well as a passion for providing high-quality care to individuals in need.

Key Responsibilities:
  1. Coordinate and manage the delivery of care services to clients in the community
  2. Conduct initial assessments and create personalized care plans for each client
  3. Liaise with clients, their families, and other healthcare professionals to ensure the best possible care is provided
  4. Schedule and assign care staff to clients based on their needs and preferences
  5. Monitor and review care plans regularly to ensure they are meeting the clients' needs
  6. Maintain accurate and up-to-date records of clients' care and progress
  7. Provide support and guidance to care staff to ensure they are delivering care in line with company policies and procedures
  8. Participate in on-call duties and provide support to care staff as needed
  9. Assist with recruitment and training of new care staff
  10. Collaborate with the management team to continuously improve the quality of care services provided
Requirements:
  1. Previous experience in a similar role within the healthcare sector
  2. NVQ Level 3 in Health and Social Care or equivalent
  3. Strong organizational and time management skills
  4. Excellent communication and interpersonal skills
  5. Ability to work independently and as part of a team
  6. Knowledge of CQC regulations and standards
  7. Proficient in Microsoft Office and other relevant computer systems
  8. Valid UK driver's license and access to a vehicle
