We are looking for a Care Coordinator to join our team in North East Surrey, on a full time basis. In return you will receive £25,000 per annum.

The role will include approximately 20 hours per week of delivering care, alongside your Care Coordination role. This will make up to 37.5 hours per week working week.

As our Care Coordinator, you will:

Ensure seamless, organised and effective scheduling and care delivery
Review care plans
Be instrumental in the onboarding of new Clients
Supervise the day-to-day community operations, whilst managing the community care team.
Carry out on-call duties outside working hours as agreed, be willing to step in to cover emergencies, and ensure regulatory requirements are met
Deliver care when required to meet the needs of the business
This role is about more than just rota management. You will be responsible for supporting the Manager with Business Growth and ensuring we are always delivering the highest possible standard of care.

Skills and experience of our Care Coordinator:

Full UK driving license with access to a car is essential;
NVQ Level 2 in Health and Social Care and looking to work towards an NVQ Level 3
A minimum two years care experience and minimum of one year's experience in a Care Coordinator role.
Good people management skills
Able to work as part of a team as well as under own initiative
Highly organised and able to prioritise.
Must have Good IT skills and know how to use Outlook, Word and Excel
Good oral and written communication skills
Knowledge of domiciliary care provision and CQC regulations is essential.

Wallington, Carshalton, Sutton, Cheam, Banstead, Chipstead, Coulsdon, Whyteleafe, Kenley, Caterham, Chaldon, Warlingham, Woldingham, Purley and Sanderstead.



Full UK driver's licence, own vehicle and flexible availability is essential. You must be available to work some evenings and every other weekend.
