Job description: Are you a highly organised, people-focused professional with a passion for making a difference? Candlelight Live-In Care is looking for a motivated Care Coordinator to join our team. We support vulnerable people to continue living in their own homes; this is a rewarding role. As our Care Coordinator, you will be the key link between our clients and care workers, ensuring smooth service delivery.

Role as Care Coordinator
  1. Responsible for allocating Clients and Carers and communicating these allocations through booking confirmations and rotas.
  2. Be the point of contact for Clients and Carers regarding their rotas.
  3. Maintain accurate and up-to-date information on Access.
  4. Develop good relationships with carers, liaise with clients, partners, stakeholders, and commissioners professionally, and deliver excellent customer service.
  5. Assist the Office team with onboarding new Clients and Carers onto the system and digital files.
  6. Provide support via telephone and email to ensure the service runs smoothly and efficiently.
  7. Work as part of the Candlelight Live-In Care team, supporting team goals and fostering a collaborative environment.
  8. Promote and uphold Candlelight values of independence, choice, dignity, respect, and the desire to keep Clients at home for as long as possible.
  9. Build strong personal relationships and influence Carers and Clients positively.
  10. Be an excellent listener, especially to those with communication difficulties or whose first language is not English.
  11. Communicate effectively both verbally and in writing.
  12. Be resilient, compassionate, professional, and a team player.
  13. Adapt to a fast-paced, changing environment with a can-do attitude.
  14. Possess competent IT skills, attention to detail, and a commitment to quality service.
  15. Manage time effectively, prioritize tasks, and maintain work-life balance.
  16. Preferably have 1 year of experience in home care.
