Job Title: Care Coordinator

We are looking for a compassionate and highly organized Care Coordinator to join our team. The successful candidate will be responsible for managing care schedules, liaising with clients and care workers, and ensuring the smooth day-to-day operation of our care services. This is a vital role that requires excellent communication, problem-solving, and time-management skills.

Key Responsibilities:
  1. Scheduling & Coordination: Create and maintain staff rotas, ensuring efficient allocation of care workers to meet client needs
  2. Client & Staff Liaison: Act as the main point of contact for clients, their families, and care staff, addressing concerns and ensuring high-quality care delivery
  3. Compliance & Record-Keeping: Ensure all care plans, risk assessments, and client records are up to date and compliant with regulatory standards
  4. Recruitment & Onboarding: Assist in recruiting, training, and supporting new care workers, ensuring they meet company and CQC standards
  5. Quality Assurance: Conduct spot checks, supervisions, and client reviews to maintain high standards of care
  6. Emergency Support: Handle last-minute changes, staff absences, and emergency situations to ensure service continuity
  7. Reporting & Communication: Collaborate with management to report on service delivery, staffing, and escalate concerns when necessary
Requirements:
  • Previous experience as a Care Coordinator or in a similar role within domiciliary care
  • Minimum Level 3 qualification in Health and Social Care
  • Excellent organizational and time-management skills
  • Strong communication and interpersonal abilities
  • Knowledge of CQC regulations and compliance standards
  • Proficiency in scheduling software and Microsoft Office
  • Ability to work under pressure and manage multiple priorities
  • Full UK driving license and access to a vehicle (preferred)
