Bury St Edmunds Primary Care Network (PCN) is looking for a motivated and compassionate care coordinator to join our growing team on a full-time basis. This is an exciting opportunity to play a key role in supporting patients across our practices by ensuring they receive the right care, at the right time, from the right service.

Main duties of the job
  • Work closely with GPs, nurses, and other healthcare professionals to support patients with complex health and social care needs.
  • Help patients navigate the health and care system, ensuring they are connected with the most appropriate services.
  • Act as a point of contact for patients and carers, providing personalised support.
  • Coordinate care for people with long-term conditions, frailty, and those requiring proactive management.
  • Contribute to improving patient experience and outcomes within the PCN.
