A PCN in Bury St Edmunds is seeking a full-time Care Coordinator to support multiple GP practices and ensure optimal patient care. The role involves working closely with patients and the multidisciplinary team, providing care coordination for vulnerable populations while balancing organizational needs.
Bury St Edmunds PCN has an opportunity for a care coordinator to join our primary care network (PCN), supporting five GP practices across the Bury area. The role is full-time, 37.5 hours per week, primarily on Monday and Friday between 08:30 and 18:00, with some weekend hours as needed.
The role involves working closely with practices, the multidisciplinary team (MDT), and the wider PCN to ensure patients receive optimal care. Responsibilities include working on requirements for the PCN Direct Enhanced Service Contract (DES).
Our care coordinators proactively identify and support vulnerable populations, including the frail/elderly and those with long-term conditions, providing care coordination and navigation across health and social care services.
Ideal candidates will have experience in care planning, strong communication skills, and excellent organizational abilities.
Bury St Edmunds Primary Care Network includes five surgeries serving approximately 67,000 patients in the town and surrounding villages.
