Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. Patients who are discharged from hospital into Home First will be receiving step-down intermediate care, involving community-based assessments and interventions provided in their own home to support recovery and monitoring of their health in the early days post discharge to prevent re-admission. Home First will, whererequired, provide acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living.

The focus of this role is providing prompt and efficient co-ordination and administration support to a multi-disciplinary team (MDT).
You will be the point of contact for service users, their families, and carers when information on their crisis and short-term care support is required. This will include enquiries about anticipated visits, providing updates, and listening to their concerns.
A key element of your duties will be processing and administering referrals using criteria guidelines and seeking advice from relevant team members when required. You will sign-post these referrals to MDT members and other services as appropriate while monitoring, tracking, and reporting on progress within the patient pathway. This will involve escalating any issues or delays to the Operational Team Lead for appropriate action.
