A local healthcare provider in Bexleyheath offers an exciting opportunity for a Care Coordinator to support clinical staff and manage administrative duties. The ideal candidate will have strong organisational and communication skills, a commitment to patient care, and relevant experience. This role involves liaising with various stakeholders and ensuring smooth daily operations to benefit both staff and patients.
A Care Coordinatorplays an important part within the surgery, They need to have a high level of administrative experience, they also need to be caring, dedicated,and reliable and enjoy working with a wide range of people. They willparticipate fully in the running of the Practice within the scope of specifiedadministration duties detailed in the job description. You will need to provide an efficient servicefor the clinical staff and Patients. Being responsible to the Practice Managerand conducting oneself in a pleasant and business-like manner. It the successful individual with be involved in exciting new income generated projects and support other administrative role within the practice.

Oversee the day-to-dayoperation during working hours of the reception team to ensure that
staffachieve their primary responsibilities, and ensuring that the wait on thepractice
telephone lines is not excessive at any one time.

appropriate.
Maintain a good knowledge ofthe clinical systems and provide training where required to staff
and patientsto enhance patient care.
Coordinate and support managementof the daily distributions of blood results.
Coordinate and support managementof 111 & OOH reports
Coordinate and support managementof the Breast Screening DNA follow ups.
OverseeQOF and generate reports on health inequalities.

Undertake continual personaland professional development, taking an active part in reviewing anddeveloping the role and responsibilities, and provide evidence of learning areawhen required.

Establish strong working relationships with GPs andpractice teams and work collaboratively with others.

Identify opportunities and gaps in the service andprovide feedback to continually improve the service and contribute to businessplanning.
Contribute to the wider aims and objectives of the PCNto improve and support primary care.
