Somerset NHS Foundation Trust is seeking a proactive care coordination professional to join the Single Point of Access Urgent Care Team. Based in our office, you will work within a fast-paced call centre to link with community services and other organisations, delivering high‑quality, person‑centred care across a flexible rota 0800–2030, 7 days a week including bank holidays.

There is an expectation that hours are worked flexibly over a rota basis, to cover the service hours, which are 7 days a week 0800-2030 including bank holidays. The post holder will need to be able to multitask to a high level, under pressure and during difficult conversations. This is an office based role within a fast paced call centre, with a main aim of linking with other Community Services within Somerset Foundation Trust and other organisations.
