Care Coodinator

Bluebird Hounslow & Chiswick is recruiting a full-time Care Coordinator to join the team, ensuring customers receive excellent quality care and support to keep them safe and comfortable in their own homes.

Key Responsibilities

1. Ensure care experts are appropriately allocated to customers for timely and safe care, aligned with their care and support plans.

Duties

1. Liaise with the registered manager to recruit care experts with the right skills and arrange cover for sickness, absence, or holidays.
2. Accept, allocate, and process new referrals promptly.
3. Process changes to care and support plans.
4. Ensure sufficient cover for timely care delivery.
5. Monitor and optimize the allocation of care experts, supporting work/life balance.
6. Use IT systems to allocate care, considering skills, preferences, travel, and emergencies.
7. Complete rotas at least 2 weeks in advance and distribute weekly.
8. Participate in out-of-hours emergency on-call rota after full training and assessment.
9. Maintain up-to-date records and ensure confidentiality.
10. Record and report incidents and accidents.
11. Conduct general office duties and prepare reports as required.
12. Communicate effectively with customers and their representatives.
13. Collaborate with the team and other professionals to improve service quality.
14. Train new and existing care experts if qualified.
15. Work flexibly and adhere to Bluebird Care policies and procedures.

Skills and Abilities

• Knowledge of domiciliary care provision.
• Ability to work independently and as part of a team.
• Highly organized with good prioritization skills.
• Excellent communication skills, both spoken and written.

Additional Requirements

Valid right to work in the UK, a valid driving licence, and access to a car are essential.
