Job Description:

Job overview: We have a wonderful opportunity in the Older People’s Community Mental Health Team, based in the East of the city (LS7), to recruit a Band 6 Nurse or OT on a temporary or secondment basis until June. We are looking for a committed and motivated practitioner who wishes to consolidate their practice in community services and develop their skills, experience, and knowledge.

This post is a great chance for an individual with an understanding of the needs of Service Users with severe, enduring, and complex mental health needs including mood disorders, psychosis, and dementia, to further their learning and development in a supportive MDT. If the candidate is an OT professional, this role utilizes those skills in assessment and intervention. Working with older service users can be challenging, so a passion for this area is essential.

The successful applicant will demonstrate the capacity to work independently under supervision. The role offers hybrid working, with availability required during working hours to respond to caseload needs and team collaboration. Clinical supervision and mentorship will be provided, along with a personal development plan.

Main duties of the job:

  • Conduct comprehensive assessments, planning, implementation, and evaluation of care for this client group.
  • Assess and escalate risks appropriately, maintaining up-to-date electronic records.
  • Provide professional input into the Multi-Disciplinary team, working collaboratively to deliver mental health interventions.
  • Provide care coordination within the CPA framework.
  • Embrace service user involvement and demonstrate a range of therapeutic interventions.
  • Work independently in the community, requiring a full UK driving license and use of a car.

The role includes a CPD package, appraisal, supervision, and training opportunities.

Working for our organization:

Leeds and York Partnership NHS Foundation Trust (LYPFT) provides mental health and learning disability services, with links to universities, supporting research and development. Benefits include employee wellbeing support, annual leave, flexible and remote working, NHS Pension, career development, and discounts.

Applicants requiring a visa must meet sponsorship requirements, as the Trust provides sponsorship only for registered healthcare practitioners.

Detailed responsibilities:

Implement care independently, supervise staff, mentor students, and support newly qualified staff. Act as duty person for triage and referrals. Training in dementia care will be provided if needed.

Operating hours are Monday to Friday, 09:00-17:00. Part-time hours are negotiable.

Person specification:

  • Qualifications: RMN/RNLD/OT
  • Experience: Working with older people
  • Skills: Excellent communication and interpersonal skills
  • Driving license/access to a car
