To assist with the ongoing development of a custom Community Care/Support Service throughout the Buckinghamshire region, we are looking for an experienced Care Coordinator. In order for clients to continue receiving care in their homes and communities, the care coordinator arranges and provides them with resources and individualised care. Because the position moves quite quickly, it calls for someone who can stay composed and upbeat while meeting deadlines and functioning under pressure.

In addition to having a solid understanding of the Bucks area, the chosen applicant would be based in our Amersham office and work the core hours of 9 a.m. to 5 p.m., Monday through Friday, with weekend availability required for on-call cover.

A care coordinator’s primary responsibilities and duties include:
  • Make sure that our customers receive care in compliance with all applicable rules, procedures, and laws.
  • Take responsibility for the distribution of caretaker rosters to guarantee that every client’s demands are satisfied.
  • Assess prospective clients to determine their specific care requirements.
  • Reviews of current care plans and reevaluations of current clients
  • Communicate with all necessary family, medical, professional, and other resources.
  • Deliver top-notch customer service to all of the company’s internal and external users.
  • Conduct prompt spot checks, supervisions, and evaluations of care personnel.
  • As a member of a rotating team, provide on-call and after-hours service cover.
  • Continue to have strong and precise communication between management and employees.

  • Post-interview, staff recruitment files are current.
  • Work flexibly to fulfil the Homecare service’s operating requirements and, if necessary, provide care in the community.
  • If necessary, pick up and drop off employees at different service users.
