Job overview

The North Community Mental Health Team in Trafford is an integrated health and social care service that strives to provide quality recovery-based support to people with complex and/or enduring mental health problems. Under the line management and clinical supervision from within the team, the post holder will maintain and coordinate timely and appropriate interventions for a caseload of service users living in the community within the framework of CPA, in line with relevant clinical pathways as outlined within the CMHT operational procedure.

Main duties of the job

The post involves care coordination of a caseload, assessing service users' mental health and risks, and managing cases according to the CPA policy. It requires respecting the individuality, values, culture, and religious diversity of clients and colleagues, contributing to a service sensitive to these needs.

Person specification

  • Knowledge of the needs of service users and carers.
  • Degree or similar RMN qualification.
  • Experience in care planning.
  • Older Adult experience, inpatient or community.
  • Experience in line management and clinical supervision.
  • Ability to work within an MDT, both as part of a team and autonomously.
