A leading healthcare provider is seeking a Care Co-Ordinator to join their supportive team. The role involves managing patient registrations and reviews, ensuring care quality, and working closely with healthcare professionals to enhance patient experiences within the PCN. Ideal candidates will have a compassionate nature, relevant healthcare experience, and a Level 2 Diploma in Health and Social Care.
We are currently looking for a Care Co-Ordinator to join our team. The hours are variable and can be between 22 hours to 35 hours per week, days and times to be agreed. Working pattern is generally fixed though some flexibility will be needed to attend PCN meetings when required.
The successful candidate will join an established group of Care Co-Ordinators within our PCN.
Within Parkgate Medical Centre you will ensure that patients receive the best care possible. Your role is a key part of our growing and progressive PCN, working to provide an enhanced care experience with a view to improving the service.
We are seeking an enthusiastic, patient focused and highly motivated individual to work within our supportive team.
Parkgate Medical Centre has around 6500 patients. We also have a branch surgery which is located in Thorogate.
We are a small, friendly and supportive team consisting of Doctors, Nurses, HCA's, administrative support and PCN roles. We are easily accessible by car or public transport.
This list is not exhaustive:
Enhanced Care Homes - managing new registrations and leavers, arranging medication reviews and managing quarterly reports, liaising with the Care Home and arranging MDT meetings.
Learning Disability - arranging annual LD reviews and ensuring screening is maintained.
Dementia Reviews - arranging reviews, monitoring carers register, identifying carers and providing support.
Early Cancer Diagnosis - ensuring the full workflow is completed for example, monitoring patients for review/screening and maintaining accurate records. Following up non responders and sending out screening invites. Overseeing the process for FIT tests and working with C The Signs.
Pro-active Care - Work alongside the lead nurse in identifying patients eligible for pro-active care.
Carrying out agreed processes to ensure QOF targets are met in line with expectations. Maintain patient information and read codes as appropriate. Maintain patient recall system in line with practice agreements. Undertake clinical searches and interrogate data as appropriate.
Assisting with enhanced service claims and cross checking data.
Work with practice staff on other areas such as the seasonal flu vaccination service and annual health screening invites.
Extraction and collation of information from System One and potentially other areas to provide to PCN Manager.
Working with PCN Manager and Lead GPs to understand exactly what is required to meet Practice and PCN requirements.
