Job description

Active Care Group’s Care in the Home division continues to grow, providing safe, reliable, and outcome-based care delivered by our exceptional support workers, clinicians, and management team.

Please note that we are unable to offer sponsorship for this role. We will not consider candidates who require a certificate of sponsorship to work in the UK.

We have a wonderful opportunity to join our friendly team of Experienced Care Coordinators supporting our Care in the Home division. You will provide premium customer service, coordinate, and schedule care packages to ensure continuity of care for our clients in their own homes.

Our services support children, young people, and adults with conditions such as brain injury, spinal cord injury, neurological and neurodegenerative diseases, respiratory and gastrointestinal conditions, including percutaneous endoscopic gastrostomy (PEG) feeding.

What you'll be working:
  1. 9am until 5:30pm
  2. Part of the on-call rota, Monday to Thursday or Friday to Monday
What you'll be doing:
  1. Making inbound and outbound calls to clients and workers to check welfare and shift coverage
  2. Updating the Team Leader on any critical issues within the team daily
  3. Ensuring compliance with company policies and procedures, maintaining standards of regulatory compliance including continuity of care, safeguarding, and handling complaints
  4. Communicating effectively with staff at all levels, service users, and the public, especially when handling complex or sensitive information
  5. Developing innovative ideas and supporting a flexible, creative, and responsive workplace
  6. Delivering best practices and ensuring workloads are managed efficiently
  7. Representing the company positively and maintaining relationships with stakeholders
What you'll have:
  • Experience as a Care Coordinator
  • Experience with booking and phone systems
  • Conflict management skills
  • A strong work ethic and a flexible, dynamic approach
  • Ability to work independently and as part of a team, with creative thinking
  • Excellent communication and interpersonal skills
  • Willingness to learn and develop
  • Successful candidates will undergo an Enhanced DBS check
