Active Care Group’s Care in the Home division continues to go from strength to strength, ensuring we provide safe, reliable, and outcome-based care to all clients through our exceptional support workers, clinicians, and management team.

We have a wonderful opportunity to join our friendly team of Experienced Care Co-ordinators supporting our Care in the Home division. You will provide premium customer service, coordinate and schedule care packages to ensure continuity of care for our clients in their own homes.

Our services support children, young people, and adults with conditions such as brain injury, spinal cord injury, neurological and neurodegenerative diseases, respiratory and gastrointestinal conditions, including percutaneous endoscopic gastrostomy (PEG) feeding.

What you'll be working:

  • 9am until 5:30pm
  • Part of the on-call rota, Monday to Thursday or Friday to Monday

What you'll be doing:

  • Inbound and outbound calls to clients and workers to check welfare and shift coverage
  • Updating the Team Leader on any critical issues within the team daily
  • Ensuring compliance with company policies and regulatory standards, including continuity of care, safeguarding, and handling complaints
  • Communicating effectively with staff at all levels and with service users and the public
  • Collaborating on innovative ideas, demonstrating vision, and supporting a flexible, creative workplace
  • Developing and delivering best practices, ensuring workloads are managed efficiently
  • Representing the company professionally and maintaining positive relationships with stakeholders

What you'll have:

  • Experience as a Care Co-ordinator
  • Experience with booking and phone systems
  • Conflict resolution skills in a supportive manner
  • A strong work ethic, flexibility, and a dynamic approach
  • Ability to work independently and as part of a team, thinking creatively
  • Excellent communication and interpersonal skills
  • Willingness to learn and develop
  • Successful candidates will undergo an Enhanced DBS check
