We are looking for two organised and compassionate Care Coordinators to join our friendly team at Rosedean House Surgery. These are exciting opportunities to play a vital role in supporting patients to manage their health and wellbeing, working closely with our GPs and the wider multidisciplinary team.

Job responsibilities

As a Care Coordinator, youll act as a central point of contact for patients and carers, helping them navigate health and care services, prepare for clinical conversations, and make informed choices about their care. Youll work with a defined caseload of patients, ensuring they receive timely, personalised, and coordinated support to improve their quality of life.

Key Responsibilities

  • Work with patients, families and carers to improve understanding of conditions and support personalised care plans.
  • Help patients manage their needs by arranging appointments, responding to queries, and providing clear information.
  • Enable access to self-management education, health coaching, and social support through partnership with link workers and other services.
  • Coordinate care across health and social care teams, ensuring communication and collaboration between professionals.
  • Proactively manage a caseload of patients, including those with long-term conditions, referring back to clinicians as needed.
  • Support multidisciplinary meetings and shared decision-making processes.
  • Maintain accurate and up-to-date patient records in line with governance and data protection requirements.
  • Identify unpaid carers and connect them with appropriate support.
  • Contribute to continuous service improvement and initiatives to reduce health inequalities.
