Overview

Roaming Care Co-ordinator – Band 4. Permanent, full-time position (37.5 hours per week). Site: Custom House, Ellesmere Port. Salary: £27,485 - £30,162 per annum. Closing date: 05/10/2025 23:59.

Role: Care Co-ordinator to work across the neighbourhood community services covering the West Cheshire footprint. You will be a central co-ordination point for local multi-disciplinary activity, ensuring high quality customer service. You will work alongside team and partners including GPs and the voluntary sector to support patients within the care community. This post requires travel between CWP bases and is Monday to Friday, 9-5.

Responsibilities
  • Co-ordination of multi-disciplinary meetings.
  • Management of daily meetings.
  • Liaising with multi-disciplinary team members.
  • Signposting team members and patients/carers to relevant services.
