Job description

We are seeking autonomous, innovative practitioners to work within our integrated Community Mental Health Team (South and West) in Bradford. Our service has developed innovative ways of delivering recovery-focused interventions to support individuals in their recovery journey.

The team includes Doctors, Nurses, Psychologists, Social Workers, Occupational Therapists, support workers, and peer workers, all delivering a recovery-focused service based on our Care Closer To Home initiative. The ideal candidate will have a dynamic approach to working as part of the multidisciplinary team (MDT).

The post holder will work directly with service users with severe and enduring mental health needs, supporting their recovery as a Care Co-ordinator. Excellent communication skills and enthusiasm for service development are essential.

Responsibilities include:

  1. Delivering community mental health care to a caseload of clients in line with the objectives of the Care Trust and team policies.
  2. Implementing all Trust policies and procedures as required.
  3. Receiving and accepting referrals, leading assessments, and making referrals to other professionals as needed.
  4. Managing a personal caseload of clients.
  5. Undertaking assessments, planning, intervention, and evaluation of care for designated clients, considering social, physical, and environmental factors.
