About the Team

The Extra care team, based at Hawthorn Court in Keynsham and St Johns Court in Bath, provides a step-down service in partnership with Curo Housing Association.

We support adults—often frail and elderly—in a home environment to promote independence, prevent hospital admissions, facilitate hospital discharges, and assess ongoing care needs. Our work involves collaborating with District Nursing, Social Care, and housing support colleagues to deliver person-centred care.

Our team includes a Registered Manager, Care Manager, Shift Leaders, and Support Workers, working closely with health and social care partners to provide integrated care.

About the Role

The Stepdown Support Worker helps service users live as independently as possible at home by providing personal care, basic health care, and therapy activities such as mobility practice and exercise supervision to maximise independence.

About You

Successful candidates will be well-supported, working under the guidance of a Shift Leader or Care Manager to help service users achieve their re-ablement goals as outlined in their care plans.

We are seeking one Support Worker based at St Johns Court in Bath. The role involves working on weekends and bank holidays on a roster, with shifts between 7 am and 10 pm.
