Responsibilities

• Act as keyworker for a group of residents and support admissions.
• Contribute to developing and reviewing individual care plans (including dementia).
• Provide personal and social care per care plans, promoting wellbeing.
• Uphold safeguarding, infection control and health & safety practices.
