Job summary

Responsibilities

• Provide practical and compassionate support in patients' homes, including personal care, meal preparation and medication administration.
• Undertake basic and follow-up assessments, develop care plans and risk assessments.
• Assist with triage of referrals and arrange equipment or onward referrals as required.
• Gather patient feedback to support service improvement and maintain adherence to Trust policies and procedures.

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

As a member of the Urgent Care team, you will play a vital role in supporting patients to remain safe, well and independent within their own homes and community settings. Working as part of a multidisciplinary team, you will deliver high-quality, person-centred care, ensuring care is provided in line with agreed care plans, risk assessments and professional standards.

You will provide practical and compassionate support, including personal care, meal preparation and medication administration, while carrying out clinical observations and delegated procedures. A key part of the role is recognising changes in a patient’s condition, escalating concerns appropriately and providing timely feedback to senior clinicians.

You will also support the triage of referrals, undertake basic and follow-up assessments, develop care plans and risk assessments, arrange equipment and make onward referrals where required. In addition, you will gather patient feedback to support service improvement and maintain a strong understanding of Trust policies and procedures to promote patient safety and quality care.

Additional Working Pattern: includes some weekend working
