We are currently seeking a dedicated and compassionate Care Assessor to join our team. As a Care Assessor, you will play a crucial role in ensuring the continuity of care for our clients while actively promoting our services and fostering strong relationships with clients, their families, and other healthcare professionals.

Responsibilities

As a Care Assessor, your responsibilities will include:

1. Arranging and completing care reviews.
2. Conducting new care assessments.
3. Developing person-centered care plans.
4. Providing guidance and care support to our care team.
5. General office administration tasks.
6. Participate in on-call management on a rota basis in Carlisle and surrounding areas, along with supporting our cluster branches such as Workington.
7. Option to work 37.5 hrs paid pro rata.

Requirements

To excel in this role:

1. You should possess an NVQ level 3 in Health & Social Care.
2. Excellent communication and organisational skills.
3. Experience in a similar role.
4. Car driver with access to your own vehicle.
