A regional healthcare provider in Lancaster is searching for a Cancer Patient Navigator. The role involves assisting the clinical team in managing Upper GI service for patients with suspected or diagnosed cancer. Effective communication skills are essential to help patients navigate the healthcare system. Ideal candidates will have experience in health or social care and strong administrative skills. This is mainly an on-site position with a salary range of £27,485 to £30,162 per annum.

Cancer Patient Navigator

An exciting opportunity has arisen for a dynamic, organised and motivated person to join the Gastroenterology Team. This a dynamic post within the team; the successful applicant will assist the clinical team in proactively managing Upper GI service, providing holistic care to patients referred with a suspicion of or diagnosed with cancer.

Effective communication skills are essential; in conjunction with clinical colleagues, you will help patients and their families navigate the complex health care system. Experience in health or social care would be advantageous, with evidence of being a team player whilst having the ability to work on your own within the boundaries of the job description.

As this is a pivotal role it will consist mainly of on site working.

Main duties of the job
  • Tracking the progress of cancer patients through their pathway of care including through the diagnostic phase, pursuing results and ensuring that all results are managed appropriately to avoid potential delay. Patients may present through various routes and recording and tracking must be in accordance with National Cancer Waiting Times standards;
  • Ensuring all patients receive appointments and next steps in their pathway in a timely fashion whilst overseeing any other aspect of managing the patient pathway;
  • Maintain and update relevant databases as required.
  • To support patients and their carers re-access specialist services without delay should they need to do so
  • Providing two-way support to the Cancer Navigator team members

The post holder will work with the Trust and UGI Cancer team, specifically providing support to the Clinical Nurse Specialists, MDT Coordinators and Cancer Trackers to ensure timely care for all patients in relevant specialities.

We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Our community services for adults are provided in people's homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.
