Job description

This is an exciting opportunity for applicants with previous NHS experience working within cancer services or with experience working with patients within a front-facing setting. The successful applicants will join the Urology team.

The Cancer Care Navigator (CCN) will be part of the Clinical Nurse Specialists (CNS) team and will work closely with nurses to provide a single point of contact for patients within that cancer tumour site. The role will also link with MDT Facilitators and Diagnostic Services to promote seamless coordination and timely management of patients at all points of the cancer pathway.

This role is patient-facing; working as a direct point of contact for suspected and confirmed cancer patients and their carers throughout the entire cancer pathway. It includes assisting in delivering the National Living with Beyond Cancer (LWBC) agenda.

The purpose of this role is to support the existing cancer team in supporting patients from referral, through diagnosis, treatment, and beyond, by providing safe, patient-centered services. The aim is to maximize patients' and carers' health and quality of life, and to improve healthcare quality and efficiency.

Join a Trust rated Outstanding by the CQC. South Warwickshire NHS Foundation Trust has been rated as outstanding following its latest inspection, and we are recruiting new staff to help us improve further.

We serve Warwickshire and beyond, including hospitals in Warwick, Stratford-Upon-Avon, Royal Leamington Spa, and Shipston-on-Stour, along with community services across the county. We are a progressive, expanding organization with ambitions to improve quality, integrate pathways, and increase productivity.

Our core values are: "Trusted to provide safe, effective, and compassionate care". During recruitment, you will be asked to demonstrate how you embody these values and how they influence your work. Regardless of your role, whether patient-facing or not, we all work with these values at the core.
