Our Responsive Adolescent andChildrens Home-Based Treatment Team (ReACH) Team is seeking a dynamic MentalHealth Practitioner to join our multi-disciplinary team providing urgent,recovery-focused support to individuals and their families that are experiencingan acute mental health crisis.
This role is fast-paced and arewarding role where no two days are the same. Youd be expected to workcollaboratively with children, young people, families and external agencies tooffer intensive home-based assessment, support and treatment. Assessments canbe carried out in Accident and Emergency (A&E), family homes or communitybases in the local area.
The ReACH Team is the frontline of urgent mental health care, supporting people at their most vulnerabletime and helping them stay safe and feel supported in a community setting.
The team are currently basedat Flemming Court in Castleford, however, from December 2025 the team will moveto Fieldhead Hospital.
The service runs 7-days perweek and on a shift basis, long days of 8am-8pm.
All employees of the Trust are strongly encouraged to befully vaccinated against COVID-19 to protect patients.
-Assessing children and young people in avariety of setting (e.g. Accident and Emergency Department, home visits,schools)
-Working with families and carers to implementsafety plans and collaborative care plans
-Undertaking risk assessments and reviewing riskdocumentation
-Allocated as lead clinician on young people onyour caseload to implement home-based treatment
-Support staff in the team and offer clinicalsupervision
-Support with delivering training and guidanceto the team
-Referrals to internal and external agencies foryoung people and families
-Taking on the role as shift co-ordinator
-Delivering therapeutic intervention over ashort period to children and young people
-Submitting safeguarding referrals whererequired

Thepost-holder will:
Provide assessment and treatment to childrenand young people with mental health needs;
Work autonomously and collaboratively withinthe MDT, providing consultation for other agencies/professionals;
Promote the safeguarding and welfare ofchildren and young people.
Toprovide routine and urgent assessment and interventions to young people with awide range of emotional and psychological problems who are referred tospecialist CAMHS, in keeping with evidence based practice with supervision froma senior clinician within professional guidelines and within the overallframework of the teams policies and procedures.
To support theongoing process of delivering a comprehensive CAMHS system.
To provideassessment, interventions, therapy to children, young people with the mostsevere, complex and persistent mental health problems, including those whoare severely mentally ill or at suicidal risk and where appropriate, familiesas both an autonomous practitioner and as part of a multidisciplinary team ina variety of settings to address need. This will include working with theCAPA model.
To apply a soundknowledge of safeguarding children and guidelines in clinical work.
To be committedto a quality approach for patient care.
Thepost-holder will demonstrate high levels of concentration and providesustained effort in assessment of complex family issues.
Tomanage own caseload, accessing support and guidance appropriate to individualneed.
To operate within the clinical and case managementsupervision framework.
To provide training, consultation and supervision toother service providers, partner agencies and junior members of staff asrequired regarding intensive interventions with children, young people andfamilies.
To participate in the development, audit andreview of this extension to existing service provision.
To undertake extended hours of work alongside fellowspecialist CAMHS practitioners.
To work collaboratively with all services to providecoherent and co-ordinated mental health services for children and youngpeople as well as other significant partners, ensuring timely transition andrelevant consultation, training and support to universal services andtargeted groups.
To use a range of Information Technology systems(including Rio) ensuring high quality data is collected appropriately, keptsecurely and confidentially
To participate in an agreed clinical audit programmefor the service, and ensure lessons learnt from both audit and clinicalincidents are reported.

To comply with both clinical and corporate governanceframeworks.
Tosupport the engagement of service stakeholders and users in the developmentand monitoring of service delivery and identify opportunities for serviceimprovement and redesign.
To actively promote user/carer involvement in allaspects of care planning/delivery whilst respecting the rights, capabilitiesand wishes of the individuals concerned.
To work in an innovative andchild, young person and family centred way, in a culture of continuousimprovement and learning.
Apply Trust policies and procedures in instances ofaggression, acute distress and disturbed mental health presentations.
To maintain relevant professional registration
To identify and communicate any concerns through therelevant management processes.
To demonstrate continued practice development andengage in annual appraisal.
