Are you a passionate, skilled, and compassionate Mental Health Professional looking to make a difference to children, young people, and families in the Wakefield District?

Our Responsive Adolescent and Children’s Home-Based Treatment Team (ReACH) is seeking a dynamic Mental Health Practitioner to join our multidisciplinary team providing urgent, recovery-focused support to individuals and their families experiencing an acute mental health crisis.

This role is fast-paced and rewarding, where no two days are the same. You will work collaboratively with children, young people, families, and external agencies to offer intensive home-based assessment, support, and treatment.

The ReACH Team is the frontline of urgent mental health care, supporting vulnerable individuals and helping them stay safe and supported in a community setting.

The service operates 7 days a week on a shift basis, preferably long days from 8 am to 8 pm.

Main Duties of the Job
  1. Assess children and young people in various settings (e.g., Accident and Emergency, home visits, schools).
  2. Work with families and carers to implement safety and care plans.
  3. Undertake and review risk assessments and documentation.
  4. Serve as the lead clinician for young people on your caseload, implementing home-based treatment.
  5. Support team staff and provide clinical supervision.
  6. Assist in delivering training and guidance to the team.
  7. Make referrals to internal and external agencies for young people and families.
  8. Take on shift co-ordinator responsibilities.
  9. Deliver therapeutic interventions to children and young people over short periods.
  10. Submit safeguarding referrals when necessary.
  11. Lead support for young people recently discharged from inpatient units.

Job Summary

The post-holder will:

  • Provide assessment and treatment to children and young people with mental health needs.
  • Work autonomously and collaboratively within the MDT, offering consultation to other agencies and professionals.
  • Promote safeguarding and the welfare of children and young people.

Key Result Areas

  1. Support the delivery of a comprehensive CAMHS system.
  2. Provide assessment, interventions, and therapy for children and young people with severe, complex, or persistent mental health issues, including those at risk of suicide, working with families and using evidence-based practices with supervision.
  3. Apply sound safeguarding knowledge in clinical work.
  4. Commit to high-quality patient care.
  5. Demonstrate high concentration and sustained effort in complex family assessments.
  6. Manage own caseload with appropriate support and guidance.
  7. Operate within clinical and case management supervision frameworks.
  8. Provide training, consultation, and supervision to other service providers and junior staff.
  9. Participate in service development, audits, and reviews.
  10. Work extended hours as needed, alongside specialist CAMHS practitioners.
  11. Collaborate with all services to ensure coordinated mental health care and timely transitions.
  12. Use IT systems (including Rio) for data collection, ensuring security and confidentiality.
  13. Participate in clinical audits and report lessons learned.
  14. Comply with governance frameworks.
  15. Engage stakeholders and users in service development and improvement.
  16. Promote user and carer involvement in care planning and delivery.
  17. Work innovatively and in a child-centred manner, fostering continuous improvement and learning.
  18. Follow policies on aggression, distress, and mental health incidents.
  19. Maintain professional registration.
  20. Report concerns through appropriate channels.
  21. Engage in ongoing professional development and annual appraisals.
Other Responsibilities

Adhere to Trust policies on safeguarding, confidentiality, data protection, health and safety, equality, and diversity. Support a safe, respectful, and inclusive working environment. Complete mandatory training and participate in supervision and appraisals. Maintain a current driving license and access to a vehicle if required.
