Key Responsibilities

• Assessment & Intervention: Conduct psycho-social and risk assessments, create tailored care plans, and deliver creative interventions to support mental well-being.
• Multidisciplinary Collaboration: Work closely with CAMHS, acute and specialist hospitals, police, education, and social care to provide coordinated support.
• Crisis Management & De-escalation: Use advanced de-escalation techniques to manage acute distress and reduce risk.
• Community-Based Support: Provide intensive home-based care to prevent A&E visits and inpatient stays, boost family/community integration, and build resilience and life skills in CYP.
• Shared Caseloads: Manage a small caseload within a multidisciplinary team, offering mutual support and ensuring comprehensive care delivery.

Operational Hours

This role includes evening and weekend shifts (8am–10pm, 7 days a week).
