AJ Case Management seeks an experienced brain injury case manager to cover Oxfordshire, London and Hampshire. The role combines remote and flexible working with regular client meetings, travel and multidisciplinary team collaboration.

The ideal candidate holds BABICM registration and a relevant professional qualification (nursing or allied health) with 5+ years’ case management experience. Competitive hourly rates and ongoing professional development are offered.

Qualifications

• Experience of brain injury case management.
• Minimum 5 years case management experience.
• Minimum 10 years post-qualification experience.
• Knowledge in support planning, risk assessment and management plans.
• Excellent verbal and written communication skills.

Responsibilities

• Providing case management services to clients with complex disabilities.
• Undertaking assessments of clients' needs and setting goals with clients and families.
• Provide regular updated reports to relevant parties within AJ Case Management standards.
• Ensure the clients’ well-being and safety is protected at all times.
• Support the client with appointments, specialist equipment, living/care needs and housing requirements.
