Salary: Pay rate is £13.23 per hour, £24,776 per annum (pro rata for job share)

A new opportunity has arisen in the North and East Integrated Care Area for a Health Coach working within the voluntary and community sector to improve the health and wellbeing of people at risk of developing long term preventable health conditions.
About Bodmin Way
Bodmin Way is a social enterprise owned by the local parish churches that is working to improve the wellbeing of residents in the town and surrounding villages. It works in partnership with a network of community organisations to support those who face the most challenges, with opportunities for everyone.
About Three Harbours and Bosvenna PCN
Three Harbours and Bosvenna PCN is a network of local GP surgeries providing NHS services based in Bodmin, Fowey, Lostwithiel and Par. The network provides a wide range of services across the area to meet the needs of the community.
Hosted by Bodmin Way, and funded by North and East ICA through Cornwall Voluntary Sector Forum, the postholder will be working closely with GP surgeries in Three Harbours and Bosvenna PCN, health practitioners and the wider voluntary and community sector to help people understand what’s important to them and provide support with the building blocks of good health including diet and nutrition, activity and movement money and resources and social connections.
As a Health Coach you will be using your health coaching skills to help people better manage preventable long term health conditions through lifestyle changes including access to better nutrition, increased physical activity and social connections. The Health Coach supports people to take control of their health and wellbeing by giving them time to focus on ‘what matters to me’
Your role will be to take a holistic and personalised approach to working with individuals, addressing existing issues and encouraging proactive prevention of new illnesses through lifestyle changes and adaptations. The role is varied and may include supporting self-management education, peer support, case management and facilitating group work.
This is an exciting opportunity to create positive change to people’s lives through an innovative approach – helping people create and maintain better health for themselves and their families. Health Coaching revolves around using your motivational interviewing and coaching skills to support people with lower levels of patient activation develop the knowledge, skills and confidence to manage their own health and wellbeing whilst increasing their ability to access and utilise community support offers.
