Job description

The BLOSM Youth Navigator will work as part of the BLOSM service within a multi-disciplinary team. The role involves supporting vulnerable individuals in the Emergency Department (ED) with issues such as violence, exploitation, substance use, social isolation, housing, criminality, and relationships. The Navigator will engage with children and young people both in ED and the community, employing trauma-informed practices and various communication methods to support them during difficult times.

Key responsibilities include:

  1. Providing trauma-informed support in ED and follow-up in the community where appropriate.
  2. Collaborating with professionals to manage complex cases involving vulnerable children and young people.
  3. Maintaining communication with the wider multidisciplinary team, including health, social care, police, education, and voluntary sectors.
  4. Developing and delivering relevant training to healthcare staff and other stakeholders.
  5. Advocating for individuals to help them achieve personal goals.
  6. Promoting health and social wellbeing through professional liaison.
  7. Building strong relationships with hospital departments such as Paediatrics and Mental Health Liaison.
  8. Supporting individuals and carers in accessing information and services, including signposting.
  9. Discussing ideas and assessments professionally, supporting individuals' interests and wishes.
  10. Maintaining accurate electronic records and participating in audits and meetings.
  11. Being flexible to work across different areas or departments to meet service needs.

We employ over 6,500 staff across Calderdale Royal Hospital, Huddersfield Royal Infirmary, community sites, health centres, and patients' homes. Our services include urgent and emergency care, medical and surgical services, maternity, gynaecology, critical care, children's services, end-of-life care, outpatient, and diagnostic imaging. We also provide community health services, including sexual health, across Calderdale.

This role is offered on a fixed-term contract, with consideration for external secondments. The successful candidate will be expected to work various shifts, including unsocial hours.
