Great opportunity for an experienced Band 6 mental health Nurse to support a community mental health team.

Opportunities have arisen for Community Mental Health Nurses (Care Co-ordinator) within both East and West CMHTs. The CMHT work with people aged 16 and over with severe and enduring mental health and social care needs.

Care co-ordinators are responsible for providing holistic assessment, interventions, risk assessments, and care plans for their caseload.

Main requirements for the role

• Assessment of health and social care needs
• Producing, monitoring, and evaluating collaborative care plans
• Risk assessment and management
• Providing evidence-based therapeutic interventions
• Providing education, advice, emotional, and psychological support
• Promoting choice and service user involvement in care planning and risk management
