Job summary

East London NHS Foundation Trust is seeking a bilingual Dementia Care Navigator/Advisor (Bengali/Sylheti) to join the post-diagnostic services in Tower Hamlets. The role sits in the Integrated Care Mental Health team comprising a Band 6 nurse and two dementia care advisors/navigators, delivering information, signposting and onward referrals to support people living with dementia and their families.

You will work with patients and carers on personalised care plans, arrange referrals to carers

Qualifications

• Bengali/Sylheti speaker preferred to support dementia care.
• Registered Mental Health Nurse (Band 6) or equivalent.
• Experience in dementia care and post-diagnostic support.
• Ability to visit patients at home in Tower Hamlets.

Responsibilities

• Provide information and signposting to patients and carers.
• Develop personalised care plans and review before discharge to GP.
• Coordinate referrals to carers centre, Adult Social Care, telecare.
• Attend Dementia Support Service MDT meetings.
• Keep notes in Rio and ensure supervision and mandatory training up to date.
