Overview

The Community Independence Service (CIS) is a growing part of Interim Care. Through our Urgent Community Response pathway, we deliver admission avoidance support, providing timely, high-quality care in patients' homes via a diverse multi-professional team.

We are seeking an experienced nurse, paramedic, or therapist who is passionate about patient care and thrives in a collaborative environment. This bi-borough role supports the Urgent Community Response pathway across Westminster and Kensington & Chelsea.

The post is ideal for someone creative, adaptable, and committed to ensuring patients receive the best care. Strong leadership skills are essential, as you will contribute to service delivery and support the wider multi-disciplinary team.

The successful candidate will work across both bases with guidance from the Borough Leads, Head of Service, and Clinical Lead. You will be jointly accountable to two managers, benefiting from their ongoing support.

Summary of the role: This is a highly skilled clinical post requiring the post holder to manage clinical risk in a fast-paced community environment, lead a borough-based clinical pathway for the urgent community response and Home First Pathway, and provide direct clinical leadership and line management for clinicians within the pathway.

Responsibilities
  • Provide overall responsibility for the safe and effective delivery of care to patients accepted into the service.
  • Establish effective communication links with community colleagues providing planned care and specialist disease-specific care, supporting integrated working and smooth handover of care.
  • Manage budgets to delegated level and participate in organisational and divisional budget monitoring.
  • Maintain safe staffing levels by monitoring rostering, minimising vacancies, and leading on recruitment and staff development.
  • Ensure new staff have comprehensive induction and ongoing personal development programmes.
  • Governance: investigate complaints and incidents, ensuring lessons are learned and implemented.
  • Workforce planning and ongoing performance management including appraisals and objective setting.
  • Manage the clinical performance of staff they line-manage, ensuring competencies align with required standards.
  • Ensure service pathway meets KPI, CQUIN, and QIPP requirements; develop and implement recovery plans as needed and report progress to senior management.
  • Develop and implement plans/projects/programmes for the service area; manage staff absence, performance, and conduct in line with HR policy.
  • Monitor sickness absence via the Trust absence reporting system and lead weekly staff meetings to maintain effective team communication and clinical excellence.
  • Analyse and interpret data to identify improvements and provide rapid support to staff as needed.
  • Communicate service information to staff, management, and external agencies in a professional manner.
  • Represent the service at internal and external meetings and provide clinical leadership to staff outside of own pathway when required.

Person Specification

Education and Qualification

  • Relevant registered Nursing/AHP qualification
  • Relevant professional body membership
  • Evidence of continued professional development to Masters level or equivalent
  • Experience working in a multidisciplinary team in a leadership role
  • Teaching/mentoring experience
  • Postgraduate training relevant to this area

Desirable

  • Understanding of current health service policies
  • Non-Medical Prescribing requirements
  • Understanding of Primary Care

Previous experience

  • Extensive clinical experience at band 7 in acute disease management and crisis care
  • Experience managing a caseload of complex patients
  • Experience leading and managing a clinical team
  • Substantial post-qualification experience in acute disease management and crisis care
  • Experience of community clinical work in inner city/urban environments
  • Experience of line management including performance management and staff development
