Do you have the skills, experience, and desire to support and influence the health of a community and its patients to tackle health inequalities. We our looking for a Nursing Associate to do that role as part of a Primary Care Network (PCN) multidisciplinary team. The post holder will mainly focus on supporting patients with Learning Disabilities and Mental Health. They will be responsible for ensuring the effective holistic provision of high-quality nursing care to people in a clinic and outreach environments who are registered with practices in Burmantofts Harehills Richmond Hill.
This exciting role with see the Nursing Associates actively contribute to a patients health and wellbeing through the delivery of holistic care, a whole-person approach which considers, and equally values, physical, psychological, public health needs, social economic, spiritual and other factors in the assessing, planning and delivery of care. Providing all relevant competencies (to be developed in line with the requirements of the service) have been achieved, she/he/they will have delegated responsibility for a pre-agreed caseload under the supervision of a nominated clinician within the PCN multidisciplinary.
About Us
The post holder will be integral to the team of Health Population Management Hub of Nursing Associates, Mental Health TNA, care coordinators, the team closely with PCN clinical pharmacists, specialist GPs, Nurses, Health and Wellbeing Coaches, Social prescribers and the wider local care partners like: Community services, hospitals and third sector partners.
Working in BHR PCN
How you will tackle health inequalities in BHR PCN, caring for patients within the population, the role holder with need to have knowledge and skills to support other common associated health conditions as well as Learning Disabilities and Mental Health. The role will also include supporting and raising awareness of cancer screening for those that are eligible and providing community NHS Health Checks. We think this is a challenging but very rewarding opportunity for a nursing associate to join our commited and dedicated team that tackle health inequalities daily. Our population have a higher prevalence of mixed mental and long-term conditions because of the nature of our more deprived communities, particularly at an earlier age. Along with our culturally diverse population, our patients are typically our hard-to-reach patients.

To work with PCN colleagues to deliver care that will be focused on patients who have, Mental Health and Learning disabilities. Or who are at risk of multimorbiditys by ensuring they are monitored and have access to receiving the physical, psychological, and social assessments.
To work under the direction/delegation of a nominated clinician, in accordance with an individual care plan, with a focus on promoting health and independence in a personalised care plan using the What matters to me approach.
To carry out specific delegated clinical and care tasks and responsibilities to a high standard and competency, under the direction and supervision of a registered nurse or other registered health professional.
To proactively monitor, and prioritise patients appointments and visits within a delegated caseload, making autonomous decisions within agreed competencies about patient care and seeking support from colleagues as appropriate.
Work in an effective and organised manner demonstrating excellent time management and organisational skills to effectively deliver person-centred care for an allocated group of individuals.
To contribute to the development of staff within the service through training and supervision.
