Location: Manchester, NH 03101

Job Type: Full-time, 40 hours per week

Sign-on Bonus: $5000 for all clinical positions

Job Description:

ITT is a community-based team providing short-term, solution-focused case management to individuals with behavioral health issues facing barriers to healthcare, with a focus on integrating services with local primary care providers. ITT Case Managers deliver direct care and case management services. A Bachelor’s degree in a related field and at least one year of experience working with individuals with psychiatric or co-occurring disorders are required.

Key Responsibilities:

  • Provide interventions to individuals with mental illness transitioning to longer-term support.
  • Use trauma-informed and evidence-based practices to engage clients, build rapport, and assist with goal setting.
  • Administer standardized assessments to clients.
  • Collaborate with clients and the multidisciplinary team to develop shared care plans.
  • Coordinate activities in the care plan, maintaining a person-centered approach.
  • Offer psychoeducation and skills training to empower clients.
  • Manage a caseload effectively, practicing good time management.
  • Follow policies on referrals, assessments, documentation, and data reporting.
  • Attend team and department meetings as needed.
  • Coordinate with community agencies and supports to ensure proper service delivery.
  • Deliver culturally relevant services, addressing disparities and promoting cultural competence among providers.

Education:

Bachelor’s degree in psychology, counseling, or a related field is required.

Skills and Competencies:

  • Knowledge of integrated healthcare practices and community systems.
  • Strong organizational and communication skills.
  • Basic computer skills for email and client management.
  • Bilingual candidates are encouraged to apply.

Qualifications:

  • Knowledge of social services, benefits systems, and referral processes.
  • Experience in case management and assessments.
  • Training in areas like CTI, Stages of Change, Motivational Interviewing, Ethics, Cultural Competence, Person-Centered Planning, or Suicidal Prevention is a plus.
  • Valid driver’s license and insurance.
  • Degree in psychology, counseling, or related field.
