Minimum Requirements
The Transfer Coordinator assesses, plans, develops, implements, and evaluates transfers for patient flow throughout the system network. They coordinate and collaborate with a multidisciplinary team to facilitate appropriate and timely patient placement and transfers. They communicate capacity management plans to nursing units, leadership, and physicians. The role requires clinical knowledge, critical thinking skills, and the ability to work in a fast-paced environment with a positive attitude towards all professional contacts.
