This is an exciting opportunity to join our Bank Support Worker Team for Community Integrated Care at our supported living service in Hemsworth.

We need empathic, patient staff to provide person-centered support to individuals who are living here.
We want everyone who accesses our services to have the opportunity to live the best life possible. You will be supporting people to work towards outcomes, enabling them to maintain independence.
You will encourage and support people to engage in the local community, clubs, cinema, shows, days out, gym, pub lunches, shopping, and activities of their personal choice, they have a real passion for being outdoors.
You will support a community within Wakefield itself, across several services there are long-standing friendships, and you will support to maintain these relationships.
We will be closely working with new partnerships, exciting new prospects, and activities for the entire community to enjoy.
Carlton Close is a supported living project for 3 young men with care and support needs 24 hours a day.
The property is a bungalow set on a quiet street. It has a large communal living space, summer house, and sensory room and all the rooms have ensuites.
