As a bank staff nurseworking within The Shakespeare Hospice Adult Community Services, you will be acore member of the multidisciplinary palliative care team. The role willinvolve working across our Hospice at Home and Day Hospice services coveringSouth Warwickshire. This will be in close collaboration with our other Hospiceand community teams, the post holder will promote holistic, person-centred carefor patients, their families and those who require specialist palliativesupport.

We provide care forpatients with life-limiting illnesses, also offering carer and bereavementsupport. Our staff nurses are key members of the team and are expected to workclosely with the Team Leaders to maintain excellent standards and developfuture service provision. The post holder will uphold our hospice values: to becompassionate, accessible, respectful and effective.

We cannot guarantee aspecific number of bank shifts per month and the role will be required on anadhoc basis in line with service needs.

Main duties of the job

Toundertake specialist patient centred assessments, using an evidence-basedpractice approach, to support patients during end of life care, or whereappropriate in maintaining and/or maximising their independence and quality oflife.

Toconfidently identify and respond to changes in the patients condition andsymptoms, adjusting care and nursing interventions accordingly and with theknowledge of the Team Leaders.

To respondappropriately in managing patient crisis/deterioration and to ensure that aneffective plan of care is in place.

Toensure that all necessary and recommended infection control precautions aretaken at all times.

Toconfidently liaise with other relevant professionals and statutory/voluntaryorganisations, as necessary, to facilitate appropriate patient care.

Toparticipate and promote advanced care planning discussions with patients andcarers, considering and respecting their priorities of care and individualpreferences at all times.
